FedEx HazMat shipping labels are OP900 / OP950 forms - anyone else get this working?

I recently received clearance from FedEx Ground for Hazardous Materials shipping.  Shipworks does support the entry of the data needed to meet FedEx requirements, but the forms are all printing on the 4x6 labels.  The FedEx required forms are both 8.5x11, one is aligned data on a preprinted/formatted form supplied by FedEx, the other is printed on plain white paper.

Has anyone been able to print these three forms (label, OP900, OP950) in the proper format on the proper printer?
