Submit a Lost/Damaged UPS from ShipWorks Shipment Claim
Submitting the below forms is for Lost or Damaged Parcels shipped through UPS One Balance only.
If your parcel was shipped using a UPS account other than a UPS One Balance account, such as an account opened outside of ShipWorks, please go to UPS.com to file your claim.
UPS One Balance is supported by UPS insurance to provide coverage for parcels shipped with ShipWorks. UPS One Balance shipments include maximum liability coverage of $100 for all shipments at no cost, know as UPS Declared Value. You have the option of declaring a higher value of the parcel at the time you process the shipment in ShipWorks and will pay an additional fee to cover the value declared. Submitting the below form will begin the process of filing an insurance claim.
Claims must be submitted within 60 days from the shipment date.
If you shipped using Next Day Air on a Friday to receive Saturday Delivery, you must have selected the Saturday Delivery option for the order or the next true business day will be Monday and will still fall under the Service Guarantee set by UPS.
Claims may take up to 3 weeks to be processed.
In the even that a claim is denied, you may submit a new claim with new or updated information.
Claims with a partially filled claims form cannot be processed. If there is an issue with your claim we will notify you via email.
To begin the UPS One Balance claim process, please fill out and submit this survey. Please be sure to fill out all required and applicable fields in order to minimize any delays with your claim.
All fields in the form must have an entry. If a field does not apply to your claim, please enter N/A into the field.
