Action Getting Disalbed

3 or 4 times in the past 2 weeks one of our actions has been disabled.   Is there some new feature that tries to disable any actions?   This is not a duplicate action it is our main upload tracking that is getting disabled and has caused late shipments uploads to Amazon before we noticed. 

We have NOT disabled this action ourselves in the Action UI.
